We are only less than 15 minutes away from downtown Chicago & less than 10 minutes away from Berwyn, Cicero, Forest Park, Melrose Park, Elmwood Park, River Forest, Maywood, Riverside, Broadview, Brookfield and Stickney. What’s the best way to get my prescription refilled?
Please contact your pharmacy first. The pharmacy will contact the doctor for you and help take care of any needed paperwork.
What insurances do you accept?
We accept MacNeal Health Providers(MHP) HMO, Aetna, Blue Cross Blue Shield, Beech Street, Cigna, Coventry, Evolutions Healthcare Systems, HealthSpring, Humana, HFN, Medicare, Multiplan PHCS, NPPN, United Healthcare, & USA MCO PPO.
Is my overnight sleep study covered by insurance?
Insurance plans vary, but sleep studies are typically covered by most insurance plans. Contact your insurance provider for information regarding your specific coverage. What does it mean to be Board Certified or a Diplomate?
Certification & Diplomate status is designed to assure the public that a medical specialist has successfully completed an approved educational program and has passed a secure examination designed to assess the knowledge, experience and skills requisite to the provision of high quality patient care.
What is an Overnight Sleep Study?
This is a diagnostic test that records measurements used to identify sleep stages and classify sleep disorders. Tiny sensors are connected to the patient’s head, face, chest and legs in order to monitor various brain and body activity, including brain waves, sleep stages, eye movements, heart rate, respiration patterns, muscle movements & oxygen in your blood. The test is neither intrusive, painful, nor uncomfortable and is entirely safe.
Can I Fall Asleep With Those Wires on Me?
Every effort is taken to make the study as comfortable as possible for the patient. The patient can change positions and the sensors can be easily disconnected in the event the patient needs to get up and use the restroom facilities during the night.
Should I Take My Regular Medication The Night of My Study?
Yes. Patients should consult with their physicians prior to discontinuing any prescription medication. However, it is critical that prior to taking part in this study, the patient disclose in the patient questionnaire all prescription medications that he/she has been taking.
What Happens During My Study?
While the patient sleeps, our highly trained technicians are carefully monitoring and recording the various critical body functions throughout the duration of the study. Whenever necessary, arrangements can be made for administering of a Continuous Positive Airway Pressure (CPAP) test where a small mask is placed around the nose in order to hold the throat open which prevents snoring and apnea events.
What Happens After My Study?
Following the patient’s study, the results are collected and immediately forwarded to Dr. Linarez for interpretation. Dr. Linarez’ office will contact you to schedule a follow up appointment. Typically, the entire process takes 1-2 weeks.
Is Parking Available?
Yes! We have free daytime and overnight parking available for our patients.
How do I send a message to my doctor?
If you need to get a message to your doctor, please call 708.613.4140. You may leave a message & all calls will be returned within 24hrs. If you need to contact the doctor directly after hours, you may dial 312-772-4140. Please do not leave urgent messages on our voicemail. In order to protect your privacy and security, we currently do not accept email messages from patients.
Will my medical information be private?
Yes. We are committed to protecting the private health information of our patients. We use health information about you for treatment, to get paid for treatment, for administrative purposes, and to evaluate the quality of care that you receive. For example, your health information may be shared with other providers to whom you are referred. In most cases, you have the right to look at or get a copy of your health information that we use to make decisions about you. If you request copies, we may charge you a cost-based fee. You also have the right to request a list of certain types of disclosures of your information that we have made. If you believe your health information is incorrect or information is missing, you have the right to request that we correct the existing information or add the missing information.
